Summary
Rolapitant inhibits BCRP, and the active metabolite of irinotecan (SN-38) is a BCRP substrate. Exposure to SN-38 could therefore rise, potentially worsening neutropenia and diarrhea. The clinical significance is not well characterised.
What this interaction means in plain words
Irinotecan liposomal and Rolapitant can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Rolapitant inhibition of BCRP efflux may increase systemic exposure to SN-38.
What to do
Monitor blood counts and for severe diarrhea during chemotherapy cycles when rolapitant is used. Manage toxicity per irinotecan dose-modification guidance.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
