Summary
Sotalol is a nonselective beta-blocker and directly antagonizes isoproterenol, a pure beta-agonist, reducing its effect on heart rate and bronchial or vascular tone. Isoproterenol can also lower potassium and provoke arrhythmias in a patient taking a QT-prolonging drug.
What this interaction means in plain words
Isoproterenol and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Isoproterenol belongs to Respiratory; Sotalol belongs to Cardiovascular.
Why it happens
Competitive pharmacodynamic antagonism at beta-adrenergic receptors; isoproterenol-related hypokalemia may add to sotalol proarrhythmic risk.
What to do
Monitor heart rate, blood pressure, ECG and potassium, and expect to need higher isoproterenol doses. Use an alternative chronotrope if the response is inadequate.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
