Summary

Lonafarnib is a sensitive CYP3A4 substrate and itraconazole strongly increases its exposure. This raises toxicity such as gastrointestinal effects, hepatotoxicity and QT prolongation.

What this interaction means in plain words

Itraconazole and Lonafarnib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Itraconazole belongs to Anti Fungals; Lonafarnib belongs to Miscellaneous.

Evidence level: High

Why it happens

Strong CYP3A4 inhibition by itraconazole reduces lonafarnib clearance.

What to do

Do not use together. Choose an antifungal without CYP3A4 inhibition.
