Summary
Itraconazole, a strong CYP3A4 inhibitor, can raise sonidegib plasma concentrations substantially. Higher exposure increases the risk of musculoskeletal toxicity such as myopathy and CK elevation, as well as other dose-related adverse effects.
What this interaction means in plain words
Itraconazole and Sonidegib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Itraconazole belongs to Anti Fungals; Sonidegib belongs to Oncology.
Why it happens
Inhibition of CYP3A4-mediated metabolism of sonidegib by itraconazole.
What to do
Avoid concomitant use and choose a non-azole or weaker CYP3A inhibitor antifungal where possible. If a strong inhibitor must be given for a short period, monitor closely for muscle symptoms and check creatine kinase.
What to ask your pharmacist
