Summary
Letermovir increases rosuvastatin exposure, raising the risk of myopathy and rhabdomyolysis. The risk is greater when cyclosporine is also given.
What this interaction means in plain words
Letermovir and Rosuvastatin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Letermovir belongs to Anti Virals; Rosuvastatin belongs to Cardiovascular.
Why it happens
Letermovir inhibits OATP1B1/1B3 hepatic uptake of rosuvastatin, increasing plasma concentrations; CYP3A4 is not a major rosuvastatin pathway.
What to do
Avoid combining with letermovir if cyclosporine is also used, or limit rosuvastatin to the labeled maximum dose. Monitor closely for muscle pain, weakness and elevated creatine kinase.
