Summary

Levoketoconazole prolongs the QT interval in a dose-related way and is contraindicated with other QT-prolonging drugs that are CYP3A4 substrates. Levofloxacin also prolongs the QT interval, so combined use raises the risk of torsades de pointes.

What this interaction means in plain words

Levofloxacin and Levoketoconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Levofloxacin belongs to Anti Bacterial; Levoketoconazole belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive prolongation of cardiac repolarization through potassium channel blockade.

What to do

Avoid the combination and choose an antibiotic without QT effects. If it cannot be avoided, obtain ECG monitoring and correct electrolytes.
