Summary

Fluoroquinolones can cause unpredictable hypoglycemia or hyperglycemia in diabetic patients. Miglitol alone seldom causes hypoglycemia, so the effect is mainly a disturbance of overall glucose control.

What this interaction means in plain words

Levofloxacin and Miglitol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Levofloxacin belongs to Anti Bacterial; Miglitol belongs to Endocrine.

Evidence level: Low

Why it happens

Fluoroquinolone effects on insulin secretion may alter glucose control; this is largely a theoretical pharmacodynamic overlap with miglitol.

What to do

Monitor blood glucose during levofloxacin therapy. Note that hypoglycemia on combined sulfonylurea or insulin therapy should be treated with glucose, not sucrose.

What to ask your pharmacist

  • Do I need to tell my doctor at the next visit?
