Summary
Both drugs prolong the QT interval, and combining them raises the risk of torsades de pointes and other serious ventricular arrhythmias. Risk is higher with hypokalemia, bradycardia, renal impairment or structural heart disease.
What this interaction means in plain words
Levofloxacin and Sotalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Levofloxacin belongs to Anti Bacterial; Sotalol belongs to Cardiovascular.
Why it happens
Additive blockade of cardiac potassium (hERG/IKr) channels leading to delayed repolarization.
What to do
Prefer a non-QT-prolonging antibiotic when feasible. If the combination is unavoidable, obtain a baseline and follow-up ECG and correct potassium and magnesium.
What to ask your pharmacist
