Summary

Levoketoconazole is a strong CYP3A4 inhibitor and also prolongs the QT interval. It can raise mefloquine levels while both drugs add to QT prolongation, which increases the risk of torsades de pointes.

What this interaction means in plain words

Levoketoconazole and Mefloquine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Levoketoconazole belongs to Endocrine; Mefloquine belongs to Anti Parasitic.

Evidence level: Moderate

Why it happens

Strong CYP3A4 inhibition raises mefloquine exposure, combined with additive QT prolongation.

What to do

Do not coadminister; use an alternative antimalarial. Remember mefloquine's long half-life when starting levoketoconazole after recent use.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
