Summary

Levoketoconazole is a strong CYP3A4 inhibitor and also prolongs the QT interval. Combined with vardenafil it can greatly increase vardenafil exposure, raising the risk of severe hypotension and QT-related arrhythmia.

What this interaction means in plain words

Levoketoconazole and Vardenafil can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Levoketoconazole belongs to Endocrine; Vardenafil belongs to Genitourinary.

Evidence level: High

Why it happens

Strong CYP3A4 inhibition reduces vardenafil clearance, with additive QT prolongation.

What to do

If used, limit vardenafil to 2.5 mg in 24 hours and monitor blood pressure and ECG. Prefer an alternative treatment where possible.
