Summary
Rucaparib is a weak inhibitor of CYP3A4 and can modestly raise exposure to CYP3A4 substrates such as linagliptin. Linagliptin has a wide therapeutic margin, so a clinically important effect is unlikely. Rucaparib may also inhibit MATE1 and MATE2-K, which could raise metformin levels.
What this interaction means in plain words
Linagliptin-metformin and Rucaparib have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Linagliptin-metformin belongs to Endocrine; Rucaparib belongs to Oncology.
Why it happens
Weak CYP3A4 inhibition by rucaparib may raise linagliptin exposure, and MATE transporter inhibition may reduce metformin renal elimination.
What to do
No routine dose change is needed for linagliptin. Monitor glycemic control and for metformin adverse effects such as gastrointestinal intolerance when rucaparib is started.
