Summary
Lomitapide is a sensitive CYP3A4 substrate, and its exposure rises markedly with CYP3A4 inhibitors. Ribociclib, a moderate-to-strong CYP3A4 inhibitor, can increase the risk of lomitapide hepatotoxicity and gastrointestinal effects. Lomitapide labeling contraindicates moderate and strong CYP3A4 inhibitors.
What this interaction means in plain words
Lomitapide and Ribociclib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Lomitapide belongs to Cardiovascular; Ribociclib belongs to Oncology.
Why it happens
Ribociclib inhibits CYP3A4 and markedly reduces lomitapide clearance.
What to do
Do not use together. If ribociclib is needed, hold lomitapide during treatment and monitor liver enzymes after restarting.
