Summary
Lonafarnib is a strong CYP3A inhibitor and is expected to raise lovastatin levels considerably. This increases the risk of statin-related muscle toxicity including rhabdomyolysis.
What this interaction means in plain words
Lonafarnib and Lovastatin are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Lonafarnib belongs to Miscellaneous; Lovastatin belongs to Cardiovascular.
Why it happens
Strong CYP3A4 inhibition by lonafarnib decreases lovastatin clearance.
What to do
Avoid coadministration; stop lovastatin or substitute a statin not reliant on CYP3A4. Warn the patient about muscle pain, weakness and dark urine.
