Summary
Lopinavir is boosted with ritonavir, a strong CYP3A4 inhibitor, which can raise vilanterol exposure. Lopinavir/ritonavir may also prolong QT and PR intervals, so the combination can increase cardiovascular adverse effects such as tachycardia and arrhythmia.
What this interaction means in plain words
Lopinavir and Umeclidinium-vilanterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lopinavir belongs to Anti Virals; Umeclidinium-vilanterol belongs to Respiratory.
Why it happens
CYP3A4 inhibition by ritonavir raising vilanterol exposure, with additive QT and beta-adrenergic cardiac effects.
What to do
Monitor heart rate, ECG and potassium, and watch for signs of beta-agonist excess. Consider an alternative bronchodilator not dependent on CYP3A4 metabolism.
What to ask your pharmacist
