Summary
Voriconazole inhibits CYP2C9, which converts losartan to its more potent active metabolite E-3174. This can raise losartan exposure and reduce formation of the active metabolite, so the antihypertensive effect may be altered and is unpredictable.
What this interaction means in plain words
Losartan and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Losartan belongs to Cardiovascular; Voriconazole belongs to Anti Fungals.
Why it happens
Inhibition of CYP2C9 (and CYP3A4) by voriconazole reduces conversion of losartan to E-3174.
What to do
Monitor blood pressure after starting or stopping voriconazole and adjust the losartan dose to response. Consider another angiotensin receptor blocker if control is lost.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Losartan with Voriconazole?
MediHelp rates the interaction as moderate. Losartan and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken. Voriconazole inhibits CYP2C9, which converts losartan to its more potent active metabolite E-3174. This can raise losartan exposure and reduce formation of the active metabolite, so the antihypertensive effect may be altered and is unpredictable.
How serious is the interaction between Losartan and Voriconazole?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor blood pressure after starting or stopping voriconazole and adjust the losartan dose to response. Consider another angiotensin receptor blocker if control is lost.
What other medicines interact with Voriconazole?
MediHelp also lists interactions of Voriconazole with, for example, Avanafil, Butabarbital, Carbamazepine. The full list is on the Voriconazole page.
