Sevoflurane prolongs the QT interval, and maprotiline may do the same, so the risk of perioperative arrhythmia is increased. Tricyclic-type antidepressants can also increase sensitivity to arrhythmias and blood pressure swings under anesthesia.

What this interaction means in plain words

Maprotiline and Sevoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Maprotiline belongs to Central Nervous System; Sevoflurane belongs to Central Nervous System.

Evidence level: Low

Why it happens

Additive prolongation of cardiac repolarization, with antidepressant-related adrenergic effects increasing arrhythmia susceptibility.

What to do

Inform the anesthesiologist and obtain a preoperative ECG. Maintain normal potassium and magnesium and monitor the ECG and hemodynamics during and after anesthesia.
