Summary
Both drugs prolong the QT interval, and maprotiline can also cause conduction abnormalities and arrhythmias in overdose or at higher doses. Combined use raises the risk of torsades de pointes.
What this interaction means in plain words
Maprotiline and Sotalol are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Maprotiline belongs to Central Nervous System; Sotalol belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic QT prolongation through blockade of cardiac potassium channels.
What to do
Avoid the combination and choose an antidepressant with less QT effect, such as sertraline. If unavoidable, monitor ECG and electrolytes.
