Summary
Both drugs can depress the central nervous system, so drowsiness and impaired alertness may be increased. Meperidine is also serotonergic and paliperidone blocks 5-HT2A and dopamine receptors, so rare serotonin toxicity or neuroleptic malignant syndrome has been described with such combinations. Orthostatic hypotension may also be additive.
What this interaction means in plain words
Meperidine and Paliperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Meperidine belongs to Musculoskeletal; Paliperidone belongs to Central Nervous System.
Why it happens
Additive CNS depression plus possible serotonergic and dopamine-blocking pharmacodynamic effects.
What to do
Start at low doses and monitor for sedation, orthostatic hypotension, and signs of serotonin toxicity (agitation, clonus, hyperthermia) or rigidity. Reduce doses if excess sedation occurs.
What to ask your pharmacist
