Summary

Corticosteroids given with nondepolarizing neuromuscular blockers have been linked to acute quadriplegic myopathy, especially in critically ill patients ventilated for prolonged periods. Prolonged weakness can delay weaning from the ventilator and prolong recovery.

What this interaction means in plain words

Methylprednisolone and Pancuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Methylprednisolone belongs to Endocrine; Pancuronium belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic effect on skeletal muscle; steroids and neuromuscular blockade together promote muscle fiber injury and myosin loss (exact mechanism not fully established).

What to do

Use the lowest effective dose and shortest duration of the neuromuscular blocker and the steroid. Monitor neuromuscular function and creatine kinase, and assess for persistent weakness after discontinuation.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
