Summary
Rolapitant is a long-acting CYP2D6 inhibitor and can raise metoclopramide exposure. This may increase extrapyramidal effects, somnolence and other adverse effects, and the intranasal dose cannot be adjusted.
What this interaction means in plain words
Metoclopramide intranasal and Rolapitant can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Rolapitant inhibits CYP2D6, reducing metoclopramide clearance.
What to do
Avoid the combination; use an oral or injectable metoclopramide product with dose reduction, or another antiemetic. Monitor for extrapyramidal symptoms.
