Summary

Procarbazine has weak monoamine oxidase inhibiting activity and can potentiate sympathomimetic agents. Combined with midodrine, an alpha-1 agonist, this may cause severe hypertension, including a hypertensive crisis.

What this interaction means in plain words

Midodrine and Procarbazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Midodrine belongs to Miscellaneous; Procarbazine belongs to Oncology.

Evidence level: Low

Why it happens

MAO inhibition by procarbazine augments adrenergic pressor effects, adding to midodrine's alpha-1 mediated vasoconstriction.

What to do

Avoid the combination where possible. If used together, monitor blood pressure closely, including supine readings, and reduce or stop midodrine for elevated values.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
