Summary
Mifepristone, especially at the higher doses used for Cushing syndrome, can prolong the QT interval and inhibits CYP3A4 and CYP2D6. Trimipramine also prolongs QT and its levels may rise, increasing the risk of arrhythmia including torsades de pointes.
What this interaction means in plain words
Mifepristone and Trimipramine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Mifepristone belongs to Endocrine; Trimipramine belongs to Central Nervous System.
Why it happens
Additive QT prolongation combined with mifepristone inhibition of CYP-mediated trimipramine metabolism.
What to do
Prefer an alternative antidepressant where possible. If combined, obtain a baseline and follow-up ECG, correct potassium and magnesium, and consider reducing the trimipramine dose.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
