Summary

NSAIDs can blunt the diuretic and natriuretic action of loop diuretics such as torsemide. This may cause fluid retention, worsen heart failure or hypertension, and, particularly with volume depletion, contribute to acute kidney injury.

What this interaction means in plain words

Nabumetone and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Nabumetone belongs to Musculoskeletal; Torsemide belongs to Cardiovascular.

Evidence level: High

Why it happens

NSAIDs inhibit renal prostaglandin synthesis, which reduces renal blood flow and sodium excretion and so opposes loop diuretic effects.

What to do

Monitor blood pressure, weight, edema and renal function, and increase the diuretic dose if response is reduced. Use the lowest NSAID dose for the shortest time, or choose a non-NSAID analgesic.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
