Summary

Volatile anesthetics such as sevoflurane depress myocardial contractility and cause vasodilation, and beta-blockade blunts the compensatory heart rate response. Together they can produce more pronounced hypotension and bradycardia during anesthesia.

What this interaction means in plain words

Nebivolol and Sevoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Nebivolol belongs to Cardiovascular; Sevoflurane belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic cardiovascular depression from anesthetic-induced vasodilation and negative inotropy combined with beta-1 blockade.

What to do

Generally continue the beta-blocker perioperatively and tell the anesthesia team. Monitor blood pressure and heart rate closely and have vasopressors or anticholinergics available.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
