Summary

Both agents can prolong the QT interval and cause sedation, and both have anticholinergic properties. Together they raise the risk of arrhythmia, excessive drowsiness and anticholinergic effects such as constipation, urinary retention and confusion.

What this interaction means in plain words

Nortriptyline and Prochlorperazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Nortriptyline belongs to Central Nervous System; Prochlorperazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation through cardiac potassium channel blockade, plus additive sedative and anticholinergic effects; prochlorperazine may also inhibit CYP2D6 modestly.

What to do

Obtain a baseline ECG in patients with cardiac risk factors and correct potassium and magnesium. Monitor for sedation and anticholinergic effects and reduce doses if they occur.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
