Summary
Two opioids given together add to sedation and respiratory depression, which can be fatal. Both also have serotonergic or monoaminergic activity in the case of tramadol, so serotonin toxicity is a theoretical concern. Tramadol may also compete for opioid receptors, making the net analgesic effect hard to predict.
What this interaction means in plain words
Oliceridine and Tramadol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Oliceridine belongs to Musculoskeletal; Tramadol belongs to Musculoskeletal.
Why it happens
Additive mu-opioid receptor mediated CNS and respiratory depression, plus tramadol's serotonin reuptake inhibition.
What to do
Avoid routine concurrent use and prefer a single opioid. If both are needed, use the lowest doses for the shortest time and monitor respiration and sedation closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
