Summary
Opicapone inhibits COMT, and with a nonselective MAO inhibitor both main catecholamine pathways are blocked. This can cause marked increases in heart rate and blood pressure. Labeling contraindicates use with nonselective MAOIs.
What this interaction means in plain words
Opicapone and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Opicapone belongs to Central Nervous System; Tranylcypromine belongs to Central Nervous System.
Why it happens
Combined inhibition of COMT and MAO prevents catecholamine metabolism, causing excess catecholamines.
What to do
Do not co-administer. Choose an alternative antiparkinsonian adjunct if tranylcypromine is required.
