Secobarbital induces CYP3A4 and can lower oxycodone levels, reducing analgesia, and its sedative effect also adds to opioid CNS and respiratory depression. Stopping the barbiturate later may cause oxycodone levels to rise.

What this interaction means in plain words

Oxycodone and Secobarbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Oxycodone belongs to Musculoskeletal; Secobarbital belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

CYP3A4 induction by barbiturates plus additive CNS depression.

What to do

Avoid the combination where possible. If used, monitor for sedation and respiratory depression and adjust the oxycodone dose when the barbiturate is started or stopped.
