Summary

Probenecid blocks renal tubular secretion of penicillin G, raising and prolonging its plasma concentrations. This is sometimes used deliberately to boost penicillin levels, for example in neurosyphilis regimens, but can increase the risk of dose-related adverse effects.

What this interaction means in plain words

Penicillin g and Probenecid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Penicillin g belongs to Anti Bacterial; Probenecid belongs to Genitourinary.

Evidence level: High

Why it happens

Probenecid inhibits organic anion transporters (OAT1/OAT3) in the proximal tubule, reducing penicillin renal clearance.

What to do

If the combination is intentional, use established regimens; otherwise monitor for penicillin toxicity such as seizures and adjust dose in renal impairment.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
