Both drugs can injure the kidney, and systemic pentamidine is also associated with electrolyte disturbances. Together they increase the risk of acute kidney injury and subsequent accumulation of streptomycin, with greater risk of ototoxicity.
What this interaction means in plain words
Pentamidine and Streptomycin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Pentamidine belongs to Anti Parasitic; Streptomycin belongs to Anti Bacterial.
Why it happens
Additive nephrotoxicity with secondary reduction in streptomycin clearance.
What to do
Avoid concurrent use when possible; otherwise monitor serum creatinine, electrolytes and hearing, and adjust streptomycin dosing to renal function.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
