Summary

Both drugs can prolong the QT interval, so combined use raises the risk of ventricular arrhythmias including torsades de pointes. Voriconazole may also raise perphenazine concentrations through CYP2D6 inhibition.

What this interaction means in plain words

Perphenazine and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Perphenazine belongs to Central Nervous System; Voriconazole belongs to Anti Fungals.

Evidence level: Low

Why it happens

Additive QT prolongation, with possible voriconazole inhibition of perphenazine metabolism.

What to do

Check ECG and electrolytes at baseline and during co-therapy. Reduce the perphenazine dose or choose an alternative if QTc becomes prolonged.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
