Summary

Procarbazine has MAO-inhibiting activity, so combining it with a tricyclic may lead to serotonin syndrome or hypertensive reactions. The risk persists for some time after stopping an MAOI.

What this interaction means in plain words

Procarbazine and Trimipramine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Procarbazine belongs to Oncology; Trimipramine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

MAO inhibition by procarbazine plus monoamine reuptake inhibition by trimipramine leads to excess serotonergic and adrenergic stimulation.

What to do

Avoid the combination; allow about 14 days between stopping procarbazine and starting trimipramine, and vice versa as labeled. Use an alternative antidepressant in patients on procarbazine.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
