Summary
Quizartinib markedly prolongs the QT interval and is labeled with a boxed warning for torsades de pointes. Adding ranolazine, which also lengthens the QT interval, increases the arrhythmia risk further.
What this interaction means in plain words
Quizartinib and Ranolazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Quizartinib belongs to Oncology; Ranolazine belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic QTc prolongation; ranolazine is also a weak CYP3A inhibitor and quizartinib is a CYP3A substrate.
What to do
Avoid the combination where an alternative antianginal is feasible. If used together, monitor ECGs frequently, keep potassium and magnesium normal, and reduce or hold quizartinib according to its labeled QTc thresholds.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
