Summary

Strong CYP3A4 inhibitors such as ritonavir-boosted saquinavir can markedly raise silodosin concentrations, and strong CYP3A4 inhibitors are contraindicated with silodosin. Increased exposure raises the risk of severe hypotension and syncope. Saquinavir is also a P-gp inhibitor, which may add to the effect.

What this interaction means in plain words

Saquinavir and Silodosin are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Inhibition of CYP3A4 and P-glycoprotein reduces silodosin clearance.

What to do

Avoid the combination; choose an alternative alpha-blocker or antiretroviral regimen.
