Summary
Verapamil may increase exposure to temsirolimus and its active metabolite sirolimus, raising the risk of mTOR-related toxicity. Intravenous temsirolimus is less affected by gut metabolism, so the effect may be smaller than with oral mTOR inhibitors.
What this interaction means in plain words
Temsirolimus and Verapamil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Temsirolimus belongs to Oncology; Verapamil belongs to Cardiovascular.
Why it happens
Inhibition of CYP3A4 metabolism of temsirolimus and its metabolite sirolimus.
What to do
Prefer a non-inhibiting alternative to verapamil where possible. If coadministered, monitor for stomatitis, cytopenias, hyperglycemia, lipid abnormalities and pneumonitis.
