Mécanisme d'action
Targets the A1-domain of von Willebrand factor (vWF) and inhibits the interaction between vWF and platelets, reducing vWF-mediated platelet adhesion and platelet consumption.
Posologie adulte habituelle (résumé de la notice)
aTTP, first day: 11 mg bolus intravenous injection at least 15 minutes prior to plasma exchange, then 11 mg subcutaneous injection after completion of plasma exchange on day 1
Grossesse et allaitement
- Grossesse
- Unknown No human data. It raises bleeding risk, so mother and fetus or newborn may bleed, especially around delivery. Guinea pig studies showed no developmental harm. Use if needed for TTP; monitor mother and newborn for bleeding.
- Allaitement
- Unknown No milk data. LactMed: as a protein, milk levels are likely very low and it is probably digested in the infant gut; use with caution, especially with a newborn. Some advise not breastfeeding with acquired TTP. Weigh benefit.
Effets indésirables signalés
- Epistaxis
- Headache
- Gingival bleeding
- Fatigue
- Urticaria
- Pyrexia
Contre-indications
- Previous severe hypersensitivity reaction to caplacizumab-yhdp or any of the excipients.
Précautions particulières
- Hemorrhage: serious and fatal bleeding can occur; risk increased with underlying coagulopathies or drugs affecting hemostasis. Interrupt if clinically significant bleeding occurs; von Willebrand factor concentrate may be given to correct hemostasis.
- Withhold for 7 days before elective surgery or invasive interventions; monitor closely for bleeding after resuming.
