Mécanisme d'action
Sargramostim (GM-CSF) is a colony-stimulating factor that supports survival, clonal expansion and differentiation of hematopoietic progenitor cells, driving granulocyte-macrophage lineages (neutrophils, monocytes/macrophages, dendritic cells) and activating mature granulocytes and macrophages.
Posologie adulte habituelle (résumé de la notice)
AML, neutrophil recovery after induction chemotherapy: 250 mcg/m2/day IV over 4 hours, starting about day 11 or 4 days after chemotherapy if day 10 marrow hypoplastic with <5% blasts; continue until ANC >1500 cells/mm3 for 3 consecutive days or maximum 42 days

- Unknown Limited human data. In rabbits it increased miscarriage at about 1.3 times human exposure. If needed in pregnancy, reconstitute only with sterile water (no benzyl alcohol). Filgrastim has more pregnancy data.
- Allaitement
- Safe No data with injected use; GM-CSF is a normal milk component and is largely digested in the infant's gut. US label: do not breastfeed during treatment and for at least 2 weeks after (lower pup survival in rabbits). LactMed: unlikely to affect the infant.

Contre-indications
- History of serious allergic reactions, including anaphylaxis, to human GM-CSF such as sargramostim, yeast-derived products, or any component of the product
Précautions particulières
- Serious hypersensitivity reactions including anaphylaxis: discontinue permanently for serious allergic reactions
- Infusion-related reactions (respiratory distress, hypoxia, flushing, hypotension, syncope, tachycardia), especially with pre-existing lung disease
- Do not give within 24 hours before or after chemotherapy or radiotherapy because of severe myelosuppression risk
