Sintesi
Fluvoxamine markedly raises tizanidine concentrations, producing severe hypotension, bradycardia, profound sedation and psychomotor impairment. Tizanidine labeling contraindicates concomitant use with strong CYP1A2 inhibitors such as fluvoxamine. The effect is large and well documented.
Cosa significa questa interazione in parole semplici
Fluvoxamine and Tizanidine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Fluvoxamine belongs to Central Nervous System; Tizanidine belongs to Musculoskeletal.
Perché avviene
Fluvoxamine is a potent CYP1A2 inhibitor and blocks the principal clearance pathway of tizanidine, causing a large increase in exposure.
Cosa fare
Do not combine. Use a different antispasticity or muscle-relaxant agent such as baclofen, or an antidepressant that does not strongly inhibit CYP1A2 (for example sertraline or escitalopram).
