Summary
Antacids that alkalinize urine can reduce renal elimination of amphetamine, prolonging exposure to its active metabolite dextroamphetamine. This may increase stimulant effects such as insomnia, tachycardia, hypertension and anxiety. Aluminum hydroxide alkalinizes urine only weakly, so the effect is usually modest.
What this interaction means in plain words
Aluminum hydroxide and Lisdexamfetamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Aluminum hydroxide belongs to Gastrointestinal Tract; Lisdexamfetamine belongs to Central Nervous System.
Why it happens
Higher urinary pH increases tubular reabsorption of the weakly basic amphetamine, lowering its renal clearance; higher gastric pH may also alter absorption.
What to do
Monitor for increased stimulant effects when antacids are used regularly, and reduce the lisdexamfetamine dose if they occur. Avoid combining with strongly alkalinizing agents such as sodium bicarbonate.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
