Summary
Itraconazole raises amlodipine and atorvastatin exposure, increasing risks of hypotension and peripheral edema, and of statin myopathy. Itraconazole also has negative inotropic effects that may add to those of calcium channel blockers, which matters in heart failure.
What this interaction means in plain words
Amlodipine-atorvastatin and Itraconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-atorvastatin belongs to Cardiovascular; Itraconazole belongs to Anti Fungals.
Why it happens
Itraconazole strongly inhibits CYP3A4, reducing metabolism of amlodipine and atorvastatin, with additive negative inotropy.
What to do
Avoid the combination if possible, or reduce the amlodipine and atorvastatin doses and monitor blood pressure, edema and muscle symptoms. Consider an alternative antifungal or temporarily withholding the statin.
