Summary
Lonafarnib is a sensitive CYP3A substrate and strong CYP3A inhibitor, which can raise atorvastatin and amlodipine levels while amlodipine may modestly raise lonafarnib. This increases risk of statin myopathy, hypotension and lonafarnib toxicity.
What this interaction means in plain words
Amlodipine-atorvastatin and Lonafarnib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-atorvastatin belongs to Cardiovascular; Lonafarnib belongs to Miscellaneous.
Why it happens
Mutual CYP3A4 inhibition and competition, with strong inhibition by lonafarnib.
What to do
Avoid the combination if possible, or limit atorvastatin dose and monitor for muscle symptoms and CK. Keep lonafarnib at its starting dose, monitor blood pressure and ECG, and reduce amlodipine as needed.
What to ask your pharmacist
