Summary
Bumetanide and hydrochlorothiazide together produce strong diuresis with potential for hypokalemia, hyponatremia, hypomagnesemia and volume depletion. Volume depletion with an ARB also increases the risk of hypotension and renal function decline.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Bumetanide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Bumetanide belongs to Cardiovascular.
Why it happens
Additive diuretic effect via sequential nephron blockade and potassium wasting, with ARB sensitivity to volume depletion.
What to do
Monitor electrolytes, creatinine, weight and blood pressure, particularly during initiation. Reduce diuretic doses or supplement potassium and magnesium as needed.
