What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Sotalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Sotalol belongs to Cardiovascular.

Evidence level: High

Why it happens

Thiazide-induced electrolyte depletion potentiates the QT-prolonging action of sotalol, with additive hemodynamic effects.

What to do

Measure potassium and magnesium before and during therapy and keep them within normal range. Monitor ECG QT interval, heart rate and blood pressure, and consider an alternative diuretic strategy if electrolytes are unstable.
