Summary
Trimethoprim blocks epithelial sodium channels in the distal nephron in a way similar to amiloride, and ACE inhibitors reduce aldosterone. Together they can cause clinically significant hyperkalemia, especially in older patients, those with renal impairment or diabetes, or on high trimethoprim doses.
What this interaction means in plain words
Amlodipine-perindopril and Trimethoprim can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-perindopril belongs to Cardiovascular; Trimethoprim belongs to Anti Bacterial.
Why it happens
Additive reduction of renal potassium excretion through ENaC blockade (trimethoprim) and reduced aldosterone (perindopril).
What to do
Avoid the combination where an alternative antibiotic exists; if it must be used, check serum potassium and creatinine within the first days and again during therapy.
