Summary
Voclosporin, a calcineurin inhibitor, can cause hyperkalemia, reduced glomerular filtration and nephrotoxicity, all of which can be worsened by an ACE inhibitor. This raises the risk of hyperkalemia and acute kidney injury, particularly in patients with lupus nephritis and reduced renal function.
What this interaction means in plain words
Amlodipine-perindopril and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-perindopril belongs to Cardiovascular; Voclosporin belongs to Miscellaneous.
Why it happens
Additive effects on potassium retention and on renal hemodynamics (afferent vasoconstriction by calcineurin inhibitor and efferent dilation by ACE inhibitor).
What to do
Monitor serum potassium, creatinine and eGFR regularly, particularly after starting or changing doses, and reduce or interrupt voclosporin per label if renal function declines.
What to ask your pharmacist
