What this interaction means in plain words
Apomorphine and Paliperidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Apomorphine belongs to Central Nervous System; Paliperidone belongs to Central Nervous System.
Why it happens
Pharmacodynamic antagonism at dopamine D2 receptors, plus additive QT prolongation.
What to do
Avoid the combination; in Parkinson's psychosis use quetiapine or clozapine instead. If it cannot be avoided, monitor ECG, blood pressure and motor control.
