Summary

Chloroquine is a weak to moderate CYP2D6 inhibitor and may raise brexpiprazole exposure, since CYP2D6 is one of its two main clearance pathways. The effect is usually modest unless other inhibitors of CYP2D6 and CYP3A4 are also present, in which case exposure can rise substantially.

What this interaction means in plain words

Brexpiprazole and Chloroquine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Brexpiprazole belongs to Central Nervous System; Chloroquine belongs to Anti Parasitic.

Evidence level: Low

Why it happens

Inhibition of CYP2D6-mediated metabolism of brexpiprazole by chloroquine.

What to do

Monitor for increased sedation, akathisia and other adverse effects of brexpiprazole. Reduce the brexpiprazole dose to a quarter of the usual dose if a CYP2D6 inhibitor is combined with a strong or moderate CYP3A4 inhibitor.

What to ask your pharmacist
