Summary
Combining a loop diuretic with a thiazide produces a strong sequential nephron blockade and can cause marked diuresis, volume depletion, hypokalemia, hyponatremia and hypomagnesemia. With an ACE inhibitor, hypotension and renal impairment are more likely.
What this interaction means in plain words
Bumetanide and Hydrochlorothiazide-lisinopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.
Why it happens
Additive diuresis and electrolyte loss from blockade of the loop of Henle and distal tubule.
What to do
Monitor electrolytes, renal function, weight and blood pressure, especially in the first weeks. Reduce diuretic doses if volume depletion develops.
