Summary
NSAIDs can blunt the diuretic and natriuretic response to loop diuretics such as bumetanide. This may lead to fluid retention, worsening heart failure or hypertension, and acute kidney injury in volume-depleted patients.
What this interaction means in plain words
Bumetanide and Piroxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Piroxicam belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins reduces renal blood flow and sodium excretion, opposing loop diuretic action.
What to do
Monitor weight, edema, blood pressure and renal function, and increase the diuretic dose if response is reduced. Use the lowest NSAID dose for the shortest time.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
