Summary

Cefepime-class cephalosporins such as cefpirome can cause neurotoxicity and lower the seizure threshold, particularly in renal impairment. Together with bupropion, which also lowers the seizure threshold, the risk of seizures may be increased.

What this interaction means in plain words

Bupropion and Cefpirome can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bupropion belongs to Central Nervous System; Cefpirome belongs to Anti Bacterial.

Evidence level: Low

Why it happens

Additive lowering of seizure threshold (cephalosporin GABA-A antagonism plus bupropion's proconvulsant effect).

What to do

Keep the bupropion dose as low as possible and adjust cefpirome dose for renal function. Monitor for myoclonus, confusion or seizures.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
